What types of health insurance do you accept?

Do I need a Doctor’s referral?

Yes, you need a doctor's referral. If you have a referral form from your doctor, please bring it with you to your first appointment.  If you are unsure about your insurance coverage please contact your insurance company to inquire.  There is often a co-pay associated with physical therapy care, and this amount varies depending on your coverage and your insurance company.
